Hmm I would presume some key files are missing, did you use XboxHDM and add files? Such as a softmods provided C & E folders? You most likely need to preform a hotswap again, I suggest installing a softmod such as UnleashX or Avalaunch.

Did you try pressing, L, R, Start, and Select at the same time?

What would pressing those buttons do exactly, and for what particular softmod? News to me.

I cant remember exactly what method it is, but once u install the softmod, it boots up to the regular xbox screen, if you hit those buttons all at once, it brings you to the unleashx screen.

That's called the igr (in-game reset) and it only works when the xbox boots the modified dash. You also have to have it enabled in your modified dash settings.

To fix the xbox you need to hotswap.

If i were you i would use the error 16 tutorial, let me find it for you.

Just follow that tutorial, make sure you delete the contents of that partition before adding the dash files.
