Subtitle out of sync

ok...I added a new subtitle to my dvd movie files, and the subtitles work great. However, there are some problems in the beginning and major problems towards the end. In the beginning, there is one scene where a foreign word and someone's voice appear and speak at the same time. This causes only the subtitle telling what the word means appear, it seems that it overlapped the voice subtitle. After that scene is over, the subtitle remains the same word throughout the whole movie~!!!! I have to manually slide my dvd playing progress bar back to make the subtitles come out the way they should.
Also, towards the end, the subtitles start appearing later than they should be appearing. By the end, the subtitles appear full 3-4 seconds after the voice actors say it~!!!! Please help, thank you!!
