I had that problem too at times. The patch is supposed to be out tomorrow so I hope that fixes it. I was able to get mine working again by deleting a file. Go into
    \My Documents\My Games\Titan Quest\SaveData\Main\(Your Character Name)\Levels_World_World01.map\Normal
Sort the files by date and delete any *.que files that are above the three files map.dat, QuestToken.myw and Quest.myw. Don't delete those, just the .que file. There should just be 1 or 2 .que files above those when you sort by date if you have had the system crash. I had to redo the last quest after doing that but it didn't crash on startup anymore.

I would probably be best to make a backup of your save directory before doing that just in case.

I had one other strange problem, the audio kept cutting out when I would take to NPCs. I had to go into my sound card setup and set it to no acceleration to get it to play correctly.
