There could be a number of causes: 1) the drive is not compatible with the CD-RW; 2) the Xbox may not work with CD-RWs' lower reflectivity; 3) the disc may need to be fully erased in order to eliminate any residual information on the tracks that can appear as cross-talk.

Try: 1) recording a CD-R to test the file structure. If it works, the files are OK.
    2) Fully erase the CD-RW to reduce cross-talk to a degree. If it does not work--
    3) send the ATIP code of the disc and manufacturer's name (found in Nero InfoTool) so that we can compare the compatibility of the disc and the drive. InfoTool will also mention the firmware version of the drive; send that, too.
