Hello all I have read many threads trying to get this fixed however I have yet to find a workable solution. I recently bought a Pioneer DVR-216DBK and am trying to burn a backup of a xbox360 game using IMGburn. In the past I used my pioneer DVR-A08XLB1 and had no problems but the burns started lasting days so I assumed the drive was going bad. However now that I have the DVR-216 in when ever I try to burn a game I get this result

Does this happen only with downloaded games?
    What drivers are you using?
    Nvidia drivers or standard MS?

Standard MS and I haven't tried to burn a DL with any other program yet but it did burn on a DVD-R using Nero and that went just fine.

    Possible Solutions:
    "Service Pack 2" - Update to SP3

"PIONEER DVD-RW DVR-216D 1.06 (D:) (SCSI)" - Windows is detecting your SATA drive as a SCSI device. Update your SATA drivers with the non-windows drivers. Use EVEREST FREE EDITION or some other program to detect your SATA chipset make and model. You may need to change the SATA settings in your BIOS to IDE or SATA mode. If you have a SATA HDD, an nVidia SATA chipset and you intend to use the nVidia drivers you should disable NCQ on your HDD because there is a known issue and your PC will likely not boot again with NCQ enabled. In device manager > storage controller/IDE devices > properties > Port0/1 > and disable command queying. Do this before you update the SATA drivers. For more info see HERE.

"(Disc ID: MKM-001-00)" - Made in Singapore? If not then get the ones made in singapore for less burn errors.

"MSCE" - Download latest Pioneer DVR-216D(BK)(v1.6) firmware. Update the DVD drive with the origional Pioneer v1.6 firmware. Load the firmware into MCSE. Change Booktype and overspeed settings. Save patched formware into new folder. Load MCSE with the .exe flash program from Pioneer. Enable "same to same" check box in bottom right corner. Save patched .exe in the same folder as the patched firmware. Rename the patched firmware to the same name as the Pioneer origional firmware. Run the patched .exe flasher in the new folder with the renamed & patched firmware and the DVD drive will be patched.

Thanks for your help it is greatly appreciated. I updated to SP3 as suggested and used MCSE to modify the firmware by checking the box for bitesetting/RPC1 patch/ increase speed. I then followed the steps you suggested to flash that firmware to the drive. I checked in the drive and it still says SCSI so I am attempting to resolve the nvidia driver issue. My system configuration is as follows.

And so I am going to try and update the serial controller driver and then attempt and disable the NCQ as you recommended. I will post how it turns out. Thanks again for all the help.

The SCSI issue is not so important if you have been able to use the drive.

If the drive's firmware has been updated / patched and you are using MKM-001 from Singapore then I would try another burn of a backup.

All your other settings in ImgBurn look good:
    Layer break is good.
4x write speed is good (that's what I use).
    Booktype will be DVD-Rom automaticlly.

    Be sure to disable:
    1. Any screen saver.
    2. Monitor power off (power management in control panel).
    3. Any other prog that might try to scan / update during the burn (antivirus).
