What brand of +R discs are you using? Burn speed?
The error is basically telling you the drive doesn't like the discs.

I'm using a NexxTech +R disk, (these are absolute piece of crap disk) Burning at 4x speed which is what I typically use when burning games. I just converted the mdf file into an iso file and DVD decrypter burned it fine with no problems but I still can't get it to load on my ps2 using disk swapper. (the game is Shin Megami Tensei Nocturne )

Can't help you much on game backup but if you're using the same way that's worked before then looks like even though you could now burn the disc it's not compatibile with the ps2.
Just my experience w/ ps2 for playing dvd movie backups - can be a bit picky on media.
Plus Nexxtech uses different manufacturers - the ones you have may not be the best. Try using Taiyo Yuden, Verbatim or made in japan Sony, Fuji or Maxell.
