Use the method explained in http://www.afterdawn.com/guides/archive/dvd2vcd_with_smart_dvd2avi_tmpgenc.cfm. After making the 1st VCD mpeg, you load the KVCD template having as input mocie the mpeg you just did in page 4 (http://www.afterdawn.com/guides/archive/dvd2vcd_with_smart_dvd2avi_tmpgenc_page_4.cfm). Maybe you can use immediately the KVCD templete in step 4, but you'll need a normal mpeg to make the sound of the kvcd by multiplaxing the KVCD M1V to a normal mpeg. I think the quality will be similar using the Video-CD mpeg instead of the .D2V project to make the .M1V, too.

Ah, for the KVCD stuff use the method explsined in http://forums.afterdawn.com/thread_view.cfm/124272 , I don't know if you can apply it to the D2V project, too, Therefore it's better to you to appliy iy after you made a Video-CD mpeg-1 from the DVD (you need it to make the movie's sound, anyway).
