One thing your logs says is it doesn't like the media. Ritek is pretty good sometimes but I have had some problems with it - burning at the supported and higher speeds. Since they're listed as 8x - try that speed or even go down to 4x. Imho 12x is too fast for this disc. I would also update the firmware on your burner.
