   Im looking for a script that resembles to something like this

   http://www.ps3-themes.com/upload-theme
   
   Basically they make you upload file(s) and fill out a simple form and it adds the file's to there server and adds the information and files automatially to a section of the site.
   
   Anybody know how I can do this or no a script that can do these things?
