unlock my nokia cell phone

please help to unlock my cell phone .

Make/model phone: nokia 6061
IMEI #:010768 00 749100 4 ` 4
Country the phone is locked in: usa
Provider the phone is locked to: cingular.

1. how do i know for sure where it is locked ie cingular or att ?
2. how do i know,how many attemp still i am left with as i have already tried to unlock my hone .
