Simple: Read errors have occured during audio read

Solution: Use software that can rip CDs accurate and reports of possible errors. Setup EAC: http://www.cd-rw.org/articles/archive/mydeneaclame.cfm

Use EAC to rip tracks to .WAV format, or the whole disc to .WAV/.CUE format. Then use your favourite program to burn a copy.
