OK, I am not the best HjT log reader but I am learning. It looks like you have four toolbars, you only need one at the most. All of the "O4" stuff runs at bootup. Have you run a full virus scan as well as a full Anti-Spyware scan (using Ad-Aware and SpyBot S&D) recently while removing anything they find? Also, before running a antivirus/spyware scan I recommend you run CCleaner.
