Appears that you may have a bad batch of media. What brand & format are you using. Use Nero to show who's media manufacturer.
It says in the log the media manufacturer and type, Ricoh is usually ok. I have tried like 6 different type of disk. Cant get known of the disk that use to work. Today I semi-reformated my machine with no help.
