MDA Phone (unlock Code please!!!)

Hello,

can anyone please provide me with the information to unlock my MDA T-mobile phone. The phone is sim locked to T-mobile network only.

S/N : HT642EH01423

P/N : 99HBH043-00 T03

It will be my greatest joy if someone can provide me with the unlock code and the procedures of how to unlock it.
