Unlock Nokia 6101 please

Please help me unlock the following phone
Nokia 6101
IMEI #: 356630008174215
It is locked in the Netherlands by Vodafone.

Thanks for the fast response, but I have a (silly) problem: how do i confirm the code and do i need the spaces? I inserted the code without spaces and pressed 'send' to confirm, it replied with 'emergency calls only'. I must be doing something wrong. Could you help me once more?

Remove Your Sim Card, Power The Phone On
Press # From The (Insert Sim) Screen
Enter codes WITHOUT spaces.
Press * 3 times fast for (p)
Press * 4 times fast for (w)
Press * 2 times fast for (+)
Verify code before pressing the last #

"Phone restrictions off", it's unlocked.
"Error code" wrong code. STOP tell me.
"Not allowed" or "Can Not Undo Restriction" it's hard locked. Used up all your free tries!!

 

I tried the instructions, but after the last # (this is on the screen: #pw+048113060661436+7#), the code stays on the screen. Confirming the code with the 5-way button doesn't do anything, pressing the send button gives me the 'emergency calls only' message.
Where do I go wrong?

Insert a non-accepting sim card.When it says phone restricted, press "undo."
It will ask for a restriction code,and enter the code in to the box on screen,and then input code press ok
